Facility Evaluation Report

Licensing Program Analyst (LPA) Kimberly Lyman conducted an unannounced case management visit in conjunction with complaint visit 22-AS-20230726155846 . LPA was greeted and granted entry into the facility by Administrator Naz Safari and explained the reason for the visit.

During the course of complaint investigation 22-AS-20230726155846, LPA reviewed a recorded video provided by facility. LPA observed there is audio recording on the video. While facility is allowed to record in common areas, audio recording are not allowed in licensed facilities.

Based on the observations made during today's visit, deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. This report was discussed with the facility representative and a copy was provided as well as appeal rights. Facility representative declined to sign the report or deficiency page.
