Facility Evaluation Report
Licensing Program Analyst (LPA) Rose Ruppert made an unannounced visit to the facility today to conduct an Annual Required Evaluation. LPA was greeted and granted entry by Staff #1. During today’s visit, LPA met with Patrick O'Connell, Program Manager (PM).
The facility is a two- story, three bedroom, two-and-a-half bath residential home with an approved fire clearance of six ambulatory clients The facility currently has a census of five clients in care. LPA observed a group session with four of the five clients upon entry with a facilitator.
During today’s visit, LPA toured the facility and inspected the physical plant, including but not limited to testing all smoke detectors, testing hot water temperature in three of three client bathrooms, and testing auditory devices on all exits . The hot water temperature measured between 105.2 and 109.7 degrees Fahrenheit and all smoke detectors were operational . The fire extinguisher is charged and was serviced on February 14, 2025. The facility’s last fire drill was conducted on August 26, 2025 . LPA observed all client rooms had the required furnishings and plenty of linens.
LPA inspected the facility food supply and observed the facility retained a minimum of two days perishable and seven days non-perishable food on hand. There were two refrigerator/freezer units with a variety of items and pantries had canned goods. Emergency supplies and water were observed in the garage. Sharps and knives were locked in a safe in the garage and chemicals and hazards were locked in a storage cabinet; as well as pots and pans.
