Re: Facility is not allowing resident to leave facility, Per interviews conducted, Residents are allowed to leave facility as long as their Physicians report permits and as well as being accompanied by staff/ family or responsible party. Per staff interviews, staff stated that Residents are often taken for walks throughout the week.
Re: Facility is charging additional fees not agreed upon at admission. Per Admission Agreement, additional fees are noted that if a resident requires care between 10:00PM to 6:00AM a nighttime fee will charge Residents for each 15 minutes increments, all six resident agreements state this and all agreements are initialed and signed confirming understanding of this policy in agreement. Per interviews with staff, staff stated that they only charge for the following fee’s which are discussed upon during admission: Admission fee, Room fee, incontinence fee, Nighttime fee and Hospice fee.
Based on information gathered from investigation, it is determined the allegations Facility is restricting residents ability to have visitors, Facility is not allowing resident to leave facility, and Facility is charging additional fees not agreed upon at admission are deemed UNSUBSTANTIATED meaning that although the allegation may have happened or is valid, there is no preponderance of evidence to prove the alleged violation did or did not occur as reported.
An exit interview was conducted with Administrator Ron Wilkes and copy of report was provided.
