Facility Evaluation Report

Licensing Program Analyst (LPA) Rose Ruppert attempted an unannounced visit to the facility for an annual required evaluation. LPA arrived at the property at 8am and the door was answered by a male. LPA introduced self and the purpose of the visit and a female came to the door and stated that, This was their home." LPA inquired if there were any residents residing in the facility and the female repeated that, "This was their home." LPA asked if Christine and Ron Wilkes manage the property to which the female stated to, "Talk to them" and proceeded to shut the front door.

LPA called and spoke to Ron Wilkes, Licensee. Mr. Wilkes stated he, "Surrendered their license," in the last month after paying his licensing fees. LPA thanked Mr. Wilkes for the information and stated we would be in communication regarding the forfeiture of license. Tenants did not allow LPA entry. Due to tenants residing in the home, Licensee did not have control of property and LPA was unable to enter to confirm if residents resided there.

An exit interview was not conducted but a copy of this report will be mailed to the licensee address on file, via certified mail.
