Facility Evaluation Report

Licensing Program Analyst (LPA) made attempted visit to facility for annual inspection. Upon knocking on front door, LPA was greeted by a female tenant (who did not disclose name). Tenant stated that home had not been an active facility for a couple of years and that they were residing there as a tenant. LPA confirmed with tenant that Licensee's Christine and Ron Wilkes are the landlords and owners of home, Tenant confirmed "yes".

LPA did not observe any residents at entry way of home. Tenant did not allow LPA access into home.

LPA verified licensing fee's are current for facility. No Staff were available for signature of attempted visit.
