Facility Evaluation Report

On March 6, 2026 at 1:15 PM, Licensing Program Analyst (LPA) Edward Kim attempted to conduct an unannounced required 1-Year annual visit using the CARE Inspection Tool. LPA Kim met with Licensee Alfonso Ventura and Liberty Ventura and explained the purpose for the visit. Licensee Liberty and Alfonso stated they could not meet today for the annual visit due to personal reasons.

Licensee stated they are not operating as a facility and have zero (0) residents. They stated they do not want to forfeit their license at the current time. The annual fees are current and up to date.
