Complaint Investigation Report
On this day, LPA Arias toured the physical plant and did not observe any toxins under the kitchen sink. LPA Arias observed medications locked in a cabinet in the hallway. LPA Arias did not observe medications anywhere outside the medication cabinet. LPA observed cleaning supplies stored in the garage.
Based on interviews conducted, observation, and record review, the preponderance of evidence standard has been met. Therefore, the above allegation is found to be SUBSTANTIATED. California Code of Regulations, (Title 22, Division 6, Chapter 8), are being cited on the attached LIC 9099D.
An exit interview was conducted with Colitia Dahabreh and a copy of the report was left with the facility representative along with appeal rights.
