Facility Evaluation Report

Licensing Program Analyst (LPA) Claudia Gutierrez conducted an unannounced case management visit for the purpose of conducting a closure visit. LPA met with Licensee Olivia Hidayat and Assistant Administrator (AAD) Ricky Vu and purpose of the inspection was discussed.

On May 1, 2025, Licensee notified Community Care Licensing (CCL) via email that their last client had relocated on the same date and requested the facility be closed. During today's visit, Licensee surrendered their License.

LPA and Licensee conducted a tour of the facility and observed the following:

There are no clients in care. The facility is not providing care and/or supervision.

LPA informed Licensee facility will be closed and a closure letter sent by mail.

There are no immediately health and safety concerns, as the facility has a census of zero.

An exit interview was conducted and a copy of this report was provided at the time of exit.
