Facility Evaluation Report
Licensing Program Analyst (LPA) Claudia Gutierrez made an unannounced case management visit as Licensee notified Community Care Licensing (CCL) on March 14, 2025, of their intent to close. LPA was greeted and granted entry by Staff Julita Paulino and the purpose of the inspection was discussed. Per Staff Paulino, all four clients continue residing at the facility and relocation for clients is currently being sought by Licensee and Regional Center of Orange County.
During today's visit, LPA interviewed two of two staff present and obtained a signed Declaration (LIC855). Staff timesheets from January 2021 - June 2024 were not available for review. LPA placed a call to Licensee and left a message requesting timesheets be provided via email by Close of Business on April 7, 2025.
LPA also conducted a Health and Safety inspection and observed the facility to have electricity, water, and gas. Appliances, including: gas stove, microwave, washer, and dryer were observed to be operational. Fire extinguisher was observed to be fully charged with a service tag dated March 12, 2025. Client bedrooms were observed to have the required furnishings.The kitchen was observed to be clean and organized, and a 2-day supply of perishable and a 7-day supply of non-perishable food was observed. Medications were observed to be centrally stored and locked in a cabinet. Sharps were observed to be stored in a locked kitchen cabinet. All and any toxic chemicals, cleaning solutions, laundry toxins, and disinfectants were observed to be inaccessible to clients.
Based on observations made during today’s inspection, no deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. An exit interview was conducted and a copy of this report was provided at the end of the inspection.
