Facility Evaluation Report

This unannounced Case Management – Legal inspection is being conducted by Licensing Program Analyst (LPA) Sean Haddad for the purpose of serving an Immediate Exclusion Order to staff member Staff #1 (S1). LPA met with Lead Staff (LS) Cody Villegas and explained the reason for today’s inspection.

The Immediate Exclusion Order was explained to LS. The Order to Individual for

Immediate Exclusion from Facility letter and the Order to Licensee/Facility of Immediate Exclusion from Facility letter were both served to LS. LS stated they understood the Order and said they had no additional questions. LPA previously spoke with Administrator (AD) Kristen Newman who stated that S1 was terminated from all the licensee’s facilities, was disassociated from all of the licensee’s facilities, and will not return to any of the licensee’s facilities. LPA confirmed that S1 is not currently present at the facility and is not associated to any of the licensee’s facilities. Facility will adhere to the exclusion order and not schedule S1 for future shifts.

An exit interview was conducted, and a copy of this report and LIC811 was provided to LS.
