Facility Evaluation Report
Licensing Program Analyst (LPA) Michael Tea conducted an unannounced visit. The purpose of today’s visit was to conduct the Annual Required inspection. LPA Tea was greeted and granted entry into the facility by staff and explained the reason for the visit. Facility is licensed for 26 non-ambulatory clients. Currently there are 25 clients during today's visit. The Administrator (AD) Abdol Arastoo arrived later to assist during the visit.
LPA Tea reviewed four client files and three staff files. Client files and staff files contained most of the required documentation. Upon review of records, the facility is up to date with the required quarterly emergency disaster drill, which was last conducted on January 5, 2026. The administrator’s certificate expires April 29, 2027.
LPA Tea along with AD Arastoo toured the physical plant. LPA toured the physical plant, checked food service, and the first aid kit. The facility is comprised of two buildings. Building A is a one-story building with five client bedrooms, three bathrooms, kitchen, medication room and a dining room. Building B is a one-story building with eight client rooms, three bathrooms and a laundry area. In between the two buildings, there is shaded seating area for the clients to enjoy. Behind Building B there is a shaded patio area for residents to sit and relax as well. The kitchen was inspected. Only staff have access to the kitchen. Perishable and non-perishable food supply was checked and adequately stocked at time of visit. Emergency food and water supplies were in the kitchen. LPA observed smoke detectors/carbon monoxide in common areas and bedrooms are operational. The fire and smoke detectors are serviced and maintained by a third-party company. Fire extinguishers throughout the facility are fully charged. Client bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each client comfortably. Client bathrooms
