Facility Evaluation Report

Licensing Program Analyst (LPA) Albert Marin made an unannounced case management visit to this facility. LPA met with Administrator (AD) Marjan Arastoo and AD Abdol Arastoo and stated the purpose of this visit.

LPA toured the interior and exterior portion of the facility. During inspection, LPA observed about 13 clients either in their respective bedrooms or in common areas, and one staff member on the floor.

LPA reviewed the conditions indicated in the Non Compliance Conference initiated last May 10, 2022.

Due to time constraints, this visit needs further review and will be completed at a later time.

LPA Marin conducted an exit interview with AD Marjan Arastoo and AD Abdol Arastoo and copy of this report was left in the facility.
