Licensing Program Analyst (LPA) Celine De Perio conducted an unnannounced case management visit to the facility to review and deliver the LIC9099-D dated for 8/31/23 for complaint: 22-AS-20230630163803.
LPA was greeted and granted entry by facility administrator (AD) Jon Castro and explained reason for visit.
An exit interview was conducted and a copy of this report, and amended LIC9099-D was provided and explained.
