Facility Evaluation Report

This unannounced case management inspection is being conducted by Licensing Program Analyst (LPA) Sean Haddad for the purpose of returning the resident file of Resident #1 (R1) which LPA removed from the facility during the Case Management - Incident conducted on 07/19/22. LPA met with Administrator (AD) Matthew Nazareno and discussed the purpose of the inspection.

During today’s inspection, LPA returned R1’s resident file to AD.

Facility representative was advised that at this time further investigation may be required. An exit interview was conducted and a copy of this report was discussed with and provided to facility representative.
