Facility Evaluation Report

Licensing Program Analyst (LPA) Jessica Cho made an unannounced visit to follow up on the deficiency issued during the Annual Inspection on August 28, 2025.

Based on the review of the Type B deficiency, 87303(a), facility has complied with the terms of the Plan of Correction (POC) as per inspection of the apartment unit of Resident #1 (R1) with Maintenance Supervisor Francisco Sarabia.

An exit interview was conducted with Business Office Manager Patty Jimenez, and a copy of this report including the Letter of Deficiency Citations Cleared were provided at the end of this visit.
