Facility Evaluation Report

This unannounced site visit was made for the purpose of a case management visit. LPA, Kathrina Chin met with Melanie Washington, Executive Director. On a 9/1/2022, LPA substantiated a complaint, complaint control number 22-AS-20220826102143 , in which two deficiencies were cited on Section 87311 and Section 87468.1(a) and both deficiencies were cited as a Type A deficiency. An amended report was provided to Melanie Washington as the two deficiencies were lowered from a Type A violation to a Type B violation.

An exit interview was conducted and a copy of this report was given to Melanie Washington, Executive Director.
