Residents engaged in an altercation while in care

Licensing Program Analyst (LPA) Jenifer Tirre made an unannounced visit to the facility today and met with Business Office Coordinator Kathleen Tamondong for the purpose of delivering the findings for above allegations.

During the course of the investigation, LPA toured the facility, interviewed staff and interviewed residents as well as obtained pertinent documentation. Based on the information received from interviews, the lack of information regarding the incident, and lack of corroborating witnesses to the incident the department has found the allegation residents engaged in altercation while in care deemed UNSUBSTANTIATED. Although the allegation may have happened or is valid, there is no preponderance of evidence to prove the alleged violation did or did not occur.

An exit interview was conducted and a copy of this report was provided to facility.
