Facility Evaluation Report

Licensing Program Analyst (LPA) Joseph Alejandre made an unannounced visit to conduct a case management visit in conjunction with complaint number 22-AS-20230328105245. LPA met with Executive Director Lindsay Schroeder and explained the reason for the visit. During the complaint visit LPA observed that the facility did not have the PUB 475 poster (See Something, Say Something poster) posted in the main entry way of the facility as required by Title 22, Division 6, Chapter 8, 87468 (c)(2)(A). A technical assistance advisory note is being issued. LPA advised the Executive Director to have the PUB 475 poster posted in the main entry way by 4/7/23. The Executive Director stated she understood. An exit interview was conduced and a copy of the report provided.
