Facility Evaluation Report

On November 17, 2025, at 8:00 AM, Licensing Program Analyst (LPA) Edward Kim conducted a continuation visit for a required 1-year annual visit that was started on November 12, 2025. LPA Kim met with Executive Director (ED) Christopher Oakeson and explained the purpose of the visit.

LPA Kim conducted an audit of resident files (R1-R30), staff files (S1-S14), and medication and medication administration record. LPA observed nineteen (R1-R19) out of thirty residents did not have an updated reappraisal for 2025. LPA Kim conducted interviews for ten residents and ten staff.

A deficiency was cited during this inspection visit according to the California Code of Regulations (Title 22, Division 6, Chapter 8). LPA Kim observed nineteen residents (R1-R19) without an updated reappraisal for 2025.

An exit interview was conducted, and a copy of this report, appeal rights, LIC811, LiC811C, LIC811C, and LIC809D were provided to Executive Director Christopher Oakeson.
