Facility Evaluation Report
On July 25, 2025, Licensing Program Analyst (LPA) Brandon Lopez made an unannounced Plan of Correction visit. LPA is conducting a follow up visit to ensure the deficiencies cited during the annual inspection conducted on July 10, 2025 were corrected. LPA was greeted and granted entry into the facility by staff after explaining the purpose for the visit. Administrator (AD) Lisa Roche was notified via telephone but could not arrive to assist with today's inspection.
On today's visit, LPA observed three residents in care and three care giving staff present. LPA observed residents eating lunch in the dining room which consisted an egg sandwich, potatoes, and yogurt. LPA also conducted a tour of the facility and observed it to be free of hazards. LPA tested the hot water temperature in resident bathrooms which measured 114.4 to 116.6 degrees Fahrenheit.
Based on the observations made during today's visit, no deficiencies are being cited per the Title 22 of the California Code of Regulations. The deficiencies that were cited during the annual inspection conducted on July 10, 2025, were cleared at time of visit. An exit interview was conducted with an authorized facility representative and a copy of the report was provided.
