Facility Evaluation Report

On this day, Licensing Program Analyst (LPA) Andrea Mendivil made an unannounced visit to conduct a required annual. LPA was greeted and granted entry into the facility by staff and explained the reason for the visit.

The facility is licensed for two (2) ambulatory clients in bedroom 1 and 2 only.

Facility is a one story, 3 bedroom 2 bathroom house with work room unit for activities located behind house. Facility has no garage and a gray exterior

Licensee stated there are 0 clients currently. LPA Mendivil toured the home and confirmed no clients are present. LPA Mendivil advised Licensee to contact their LPA when they admitted their first client.

An exit interview was conducted and a copy of this report was provided.
