LPA observed a fire extinguisher near the kitchen charged and with a service date of June 26, 2026. LPA observed activity supplies in a cabinet by the living room. The resident bedrooms had all the required components and furnishings. LPA observed the bathrooms to be stocked with toilet paper, paper towels and non slip mats in the showers. The water was tested to be between 113.5 and 114 degrees Fahrenheit. LPA observed clean linens in a hall cabinet by the bathroom outside of resident bedrooms. LPA observed the medication to be in a locked closet by the same bathroom and made inaccessible to residents. LPA observed the garage to be locked and made inaccessible to residents. In the garage extra facility food is stored in a fridge and freezer as well as on shelves. LPA observed facility supplies such as toxins and chemicals to be stored in a bin in the locked garage.
