Facility Evaluation Report
On April 10, 2026, Licensing Program Analyst (LPA) Eboni Bentley made an unannounced Case Management Health and Safety check visit to follow up on a client death. LPA Bentley was greeted and granted entry by Social Worker Jeannette Fellhauer, after stating the reason for the visit. Administrator Dr. Cole Dyar was out of town. Program Director Amber Ontiveros and Clinical Director Mary Lawrence, arrived shortly to assist with the visit.
The incident is as follows: On April 9, 2026, the Department received a report that Client 1 (C1) exited the facility and passed away after being hit by a vehicle off-site on April 9, 2026 at approximately 3:15am. An Incident Report (LIC624) and Death Report (LIC624A) were later submitted to the Department. It was reported that Staff #1 witnessed the incident and Fountain Valley Police Department arrived at the scene immediately after. C1 passed away at the scene.
During today's date, LPA conducted three staff interviews, inspected the facility, and obtained the following records: Face Sheet, Resident House Rules, Treatment Plan, Physician's Report, Health & Physical (H&P) Physiatric Evaluation, Progress Notes, Centrally Stored Medication Count, medication orders, Personal Property and Valuables, and Plan of Operations.
An exit interview was conducted with Program Director Amber Ontiveros and Clinical Director Mary Lawrence, and a copy of this report was provided at the end of the visit.
