Facility Evaluation Report
On February 4, 2026, Licensing Program Analyst (LPA) Jessica Cho made an unannounced visit for the purpose of conducting the Required- 1 Year evaluation using the Care Inspection Tool. LPA was greeted and granted entry after explaining the reason for the visit to Case Manager Keesha Wright. Executive Director Dr. Cole Dyar and Clinical Director Mary Lawrence arrived on site and remained at the facility for the duration of the evaluation.
This facility is a two-story residential home comprised of three client shared bedrooms, three client bathrooms, one staff bathroom, living room, dining area, kitchen, medication office, clinical office, support staff office, and a two-car garage. The client census is three (3) and six (6) staff are on duty. Facility is operating within the conditions and limitations specified on the license. LPA toured the premises accompanied by Clinical Social Worker Melissa Baumann. LPA observed all client bedrooms had all required elements with ample lighting. All bathrooms were clean and operational. The hot water temperature measured at 106.5, 106.5, and 107.4 degrees Fahrenheit. There were a clean supply of linens in the office and garage. LPA observed ample two-day supply of perishables and seven-day supply of non-persishable food. Toxins, disinfectants, sharps, and medications were secured and inaccessible. LPA observed the fire extinguishers were mounted, charged, and serviced on October 10, 2025. The indoor temperature remains at a comfortable range. The dual functioning smoke/carbon monoxide detectors were tested and operational. The fire place was screened, and there are no bodies of water on premise. Facility maintains emergency food, water, and disaster supplies as noted on the Emergency Disaster Plan. The first aid kits are available.
