Facility Evaluation Report
On September 29, 2025, at 8:50 AM, Licensing Program Analyst (LPA) Edward Kim conducted an unannounced required 1-Year annual visit using the CARE Inspection Tool. LPA Kim met with Licensee (LI) Elizabeth Maldonado and explained the purpose of the visit.
The facility is licensed to operate for six (6) ambulatory. The facility is a single-story building located in a residential neighborhood. It consists of the following: three (3) client bedrooms, one (1) staff bedroom, one office area, three (3) bathrooms, living area, dining area, kitchen, and outside covered patio area.
LPA Kim toured indoor and outdoor of the physical plant with Direct Support Professional (DSP) Marissa Ujkic. There were no bodies of water or obstructions on the premises. There is a jacuzzi in the backyard, but there is not water in it and it is locked. The doorbell for the facility was not operable during the visit. All rooms were inspected. Beds and bedding supplies were in good condition, adequate lighting was provided, storage for each client’s personal belongings was observed. Bed linens, comforters, and bath towels were adequately stocked at the time of visit. All rooms were inspected: Client Room 1, Client Room 2, and Client Room 3. Bathrooms were found to be clean and operational. The water temperature measured between 122.7 degrees F and 131.3 degrees F. A comfortable temperature of 72 degrees F was maintained in the facility.
