Facility Evaluation Report

On September 26, 2025, at 2:30 PM, Licensing Program Analyst (LPA) Edward Kim attempted to conduct an unannounced required 1-Year annual visit using the CARE Inspection Tool. There was no one at the facility. LPA Kim rang the door bell and knocked on the door at 2:30PM and there was no answer. LPA Kim attempted to contact= through telephone the facility phone # (714)622-4316 at 2:32PM and 2:36PM. The phone rang twice and went to dial-up sounds. LPA Kim rang the door bell at 2:38 PM and knocked on the door. to follow-up information on the facility. LPA Kim attempted to contact the mobile phone # on file, but went straight to voicemail at 2:32 PM and 2:43PM. An unknown neighbor was nearby and told LPA Kim the staff and clients left around 2:00 PM.
