On April 29, 2026, Licensing Program Analyst (LPA) Eboni Bentley arrived at the facility unannounced for the purposes of conducting a case management visit. This is a follow up to a previous deficiencies and a hospice waiver request for three received by Orange County Regional Office. LPA introduced self to
Licensee/Administrator (AD) Cristina Lauguico , explained the reason for the visit, and was granted entry into the facility.
During the visit, LPA toured the facility with AD and observed no issues or concerns.
