Facility Evaluation Report

On May 13, 2025 at 1:00pm, Licensing Program Analysts (LPA) Eboni Bentley made an unannounced

required annual inspection at this facility and was greeted, granted entry by Direct Service Provider Amelita DelaCruz. Facility Administrators (AD) Laura Palomino and (AD) Isabel Ybarra were called and later arrived, at which time LPA discussed purpose of today's visit in detail.

This is a LeveI 4G Facility licensed for a capacity of 4 ambulatory clients only, providing services to Adult clients with Developmental Disabilities ages 18-59. The building is a two-story home, licensed for Residential Adult Care, with four client bedrooms, two client bathrooms, kitchen, dining room, living room, family room, staff office space/break room, one staff bathroom, backyard and attached 2-car garage.

During the visit, LPA Bentley conducted a tour of the physical plant accompanied by AD Isabel Ybarra and the following was observed: There were no bodies of water on the premises, all rooms were inspected, beds and furniture were available, lighting was provided in all rooms, and storage for the client's personal belongings. Bathrooms were operational and hot water temperatures were measured between 107.5 and 114.0 degrees Fahrenheit. Smoke and carbon monoxide detectors were tested and operational. A fully charged fire extinguisher was found near the kitchen with the last service date of June 12, 2024.

All client bedrooms were clean, well-organized with all the necessary items and storage space available. Additional bed linens and bath towels were available in hallway cabinets. The personal and staff bathrooms were checked. Toilets and water faucets worked properly, and the grab bars were secure.
