Facility Evaluation Report
On March 24, 2026, Licensing Program Analyst (LPA) Jenifer Tirre conducted an unannounced required visit using the CARE Inspection Tool. LPA was greeted by staff and granted entry after stating the purpose of the visit. Administrator Cristina Caballes arrived shortly after to help assist with the facility inspection.
The facility is a Level 3A Adult Residential Facility serving Adult clients ages 18 to 59 years old. Facility is licensed for four (4) ambulatory Clients. Currently, there are four (4) clients present during today’s visit.
Facility is a two story home with a detached garage. First floor there are four private bedrooms and two bathrooms. Second story is for Licensee and family.
During visit LPA reviewed records. Four client records and two staff records were reviewed.
Due to time constraints, a follow up inspection visit is to be continued at a later time.
An exit interview was conducted with Administrator Caballes along with copy of report provided.
