Facility Evaluation Report
Licensing Program Analyst (LPA) Taylor Simerly and Licensing Program Managers (LPM's) conducted an unannounced visit for the purpose of conducting a required one-year annual inspection. LPA entered through the Skilled Nursing side of the Facility (SNF) and was greeted and granted entry by staff and explained the reason for the visit. SNF Administrator (AD) Angelo Munoz arrived shortly to help conduct visit.
AD Munoz explained the Assisted Living (AL) portion of the facility does not have any residents and has not operated or had residents since 2020. LPA, LPM's and Administrator toured the physical plant.
During the tour, rooms and the dining room was opened by staff and observations were made. LPA did not observe any residents or any signs of any residents living in any of the resident units. The facility does have electricity and running water.
During the tour, every room that was observed was being used to store supplies for the SNF. After the tour, it was determined the facility is non-operational. AD stated that they plan to accept residents later this year and keep the license open.
LPA advised contacting the department prior to placing residents to conduct a further inspection.
No deficiency cited in this review as per Title 22 Division 6 of the California Code of Regulations.
An exit interview was conducted with AD, and a copy of this report was provided.
