The inspector’s account

On this day, Licensing Program Analyst (LPA) Kevin Saborit-Guasch made a scheduled visit to the facility at the 2870 Alanzo Lane location in Costa Mesa where the licensee has submitted a request for a CHange of Location via form LIC200 addressed to the Orange County Regional Office on July 17, 2025. LPA was greeted and granted entry by administrator Megan Parrish as facility staff was assisting with relocation furnishings and client belongings into the premises.

The fire clearance from the Costa Mesa Fire Department was provided to the Department on September 19, 2025 after a visit dated September 18. The cleared capacity is four ambulatory clients.

LPA accompanied by facility staff conducted a tour of the physical plant. The facility is a one-story house with four single bedrooms and two shared bathrooms, one of which is en-suite from one of the bedrooms. Bedrooms are equipped with the required items of furnishing relocated from the 521 Fullerton Ave location earlier on the day of the visit. Bathrooms are clean and sanitary. Water temperature was measured at 110F in both bathrooms. The lease agreement confirms the licensee has control of the property on a month-to-month basis. The anticipated date of return to the Fullerton Ave location is determined to be on or around December 1, 2026. The facility includes one swimming pool and one hot tub. The swimming pool is adequately fenced and the hot tub is equipped with a secure rigid cover.

Based on the observations conducted during today's visit, the Regional Office approves the change of location request to 2870 Alanzo Lane. All relevate information will be updated in the Licensing Information System. Updated license to be provided on Monday September 22, 2025.

An exit interview was conducted and a copy of this report was provided to a facility representative.
