Facility Evaluation Report

Licensing Program Analyst (LPA) Fred Arias attempted to conduct an unannounced visit to verify facility closure, and to ensure no clients were receiving care and supervision at the facility. LPA observed facility has a for sale sign on the front yard. LPA knocked on the front door and receive no response. There was no visible movement observed around the facility. No lights were observed to be on through the front door glass. No vehicles observed in the driveway. The neighbor next door confirmed they haven't seen anyone living at the facility but sees construction crews around dinner time entering the house from time to time. Based on LPA observation, the facility is now closed. The Department will proceed with the closure as of May 30, 2025.
