Facility Evaluation Report

Licensing Program Analyst (LPA) Fred Arias attempted to conduct an unannounced visit to verify facility closure, and to ensure no clients were receiving care and supervision at the facility. LPA observed facility is in the middle of a major remodel. There was a construction crew working at the facility. A crew member stated there's no one living in the facility. LPA observed through a window that there are additional construction projects inside the facility. Based on LPA observation, the facility is now closed. The Department will proceed with the closure as of May 30, 2025.
