Licensing Program Analyst (LPA) Fred Arias attempted to conduct an unannounced visit to verify facility closure, and to ensure no clients were receiving care and supervision at the facility. LPA observed facility is in the middle of a major remodel. The interior of the facility has been gutted. There was no one at the facility during the attempted visit. Based on LPA observation, the facility is now closed. The Department will proceed with the closure as of May 30, 2025.
