The inspector’s account

Licensing Program Analyst (LPA) Hanna Gough made an unannounced visit to the facility for the purpose of conducting the required annual inspection. LPA was greeted and granted entry by staff. LPA met with Administrators (AD) Lenette Belen and Alma Encelan and discussed the purpose of the visit.

The facility is a one story home with four client bedrooms, two staff rooms, living room, dining room, kitchen and attached two car garage. The facility appears clean, safe and sanitary. The facility has six clients in care of which two were seen at the facility watching tv and lounging in their rooms. LPA observed all client bedrooms have the required components and furnishings. LPA observed the kitchen to be clean and free of vermin. LPA observed a two day perishable and seven day nonperishable food supply on hand. LPA observed the knives to be in a locked drawer by the kitchen sink. LPA observed the emergency food and water supply to be located in the pantry. LPA observed the toxins and chemicals to be in a locked cabinet near the washer and dryer by the kitchen and made inaccessible to clients in care. LPA observed the centrally stored medication to be in locked cabinets located in the dining room. LPA observed a fire extinguisher charged and with a service date of April 8, 2025, in the dining room. LPA observed activity supplies for client use in the covered patio area. LPA observed the bathrooms to have toilet paper, hand dryers and textured shower flooring. LPA tested the water to be at 107.4 degrees Fahrenheit in the client bathroom. LPA observed a shaded seating area in the backyard for client use. LPA did not observe any debris or obstructions in the backyard. LPA observed the locked garage to be used as extra storage and is made inaccessible to clients in care. LPA and AD tested the carbon monoxide and smoke detectors, and they were found to be operational.
