Facility Evaluation Report
Licensing Program Analyst (LPA) Hanna Gough arrived at the facility to conduct a case management follow up visit on an incident report that was submitted to the Orange County Regional Office on July 10, 2025. LPA was greeted and granted entry by staff. LPA met with Administrator (AD) Lenette Belen and explained the purpose of the inspection. LPA toured the facility and observed all clients to be out at day program. There are currently four clients in care.
The incident report submitted to the Orange County Regional Office stated that on July 1 st , 2025 an eviction notice was given to Client#1 (C1) due to non-payment of rent for three months. The Department did not receive a copy of the eviction notice as of July 27, 2025. Regulation states that an eviction notice must be sent to the Department within five days of the eviction. AD informed LPA that C1 moved out of the facility on July 11 th , 2025.
Based on today’s inspection a deficiency is being noted per Title 22 Division 6 of the California Code of Regulations. An exit interview was conducted and a copy of this report along with 809-D and appeal rights were given at the time of the inspection.
