Facility Evaluation Report

Licensing Program Analyst (LPA) Claudia Gutierrez made an unannounced visit for the purpose of conducting a case management health check. LPA was greeted by Administrator (AD) Lito T Soliven and granted entry. LPA explained the reason for the visit. LPA and AD took a tour of the facility and observed the following:

This is a single story home with five bedrooms, with one being occupied by staff, and two bathrooms. One resident was observed watching tv in the living room and the two other residents were observed resting in their perspective rooms.

No deficiencies are being cited. LPA observed no health and safety violations at this time.

An exit interview was conducted and a copy of this report was left at the facility.
