Facility Evaluation Report
On today's date Licensing Program Analyst (LPA) William Vanegas conducted an unannounced visit for the purposes of conducting an annual inspection. Upon arrival LPA was greeted and granted entry into the facility by Administrator (AD). LPA explained the purpose of the visit, and began to conduct a tour of the facility accompanied by the owner Omodute Taiwo. LPA observed the following. AD has a valid Administrator certificate effective from May 16,2024 through May 15, 2026.
The facility is a once storied home with four bedrooms all of which are utilized by clients in care, the facility has two bathrooms one of which is a private bathroom and one of which is a shared bathroom. The facility also has an attached two car garage. LPA observed kitchen area to be clean and free of any mildew and debris. LPA observed there to be a gas stove, refrigerator, and a microwave in the kitchen area. All appeared to be in good repair and tested operational. LPA observed a two day supply of perishable food and a seven day supply of non-perishable food on hand. LPA observed a sufficient amount of emergency water on hand.
LPA observed all client rooms to be large enough to walk about freely, to be clean and free of any hazards or debris, and to have all required furnishings including a chest of drawers, a reading lamp, a chair, a bed, clean linens in good repair; meaning no strains or tares, and enough storage space to store personal belongings. LPA observed bathrooms to be clean and free of any mildew and debris. Water faucets and toilets tested operational. LPA observed bathrooms to have all required furnishings including grab bars, and slip resistant floor matts. Hot water temperature tested between 120.4 and 126.5 degrees Fahrenheit.
