The inspector’s account

On today's date Licensing Program Analyst (LPA) William Vanegas conducted an unannounced visit for the purposes of conducting an annual inspection. Upon arrival LPA was greeted and granted entry to the facility by facility staff. Administrator (AD) Daryl Abarquez was notified via telephone and arrived shortly after and was able to assist LPA with the annual inspection. LPA began a tour of the facility and observed the following. AD has a valid Administrator certificate valid from June 20, 2025 through October 9, 2027.

The facility is a one storied home with five bedrooms four of which are utilized for residents in care, and one of which is utilized for live in staff. The facility is equipped with an attached two car garage. LPA observed the kitchen area to be clean and free of any mildew and debris. LPA observed for the kitchen to be equipped with a gas stove, refrigerator, microwave, and a dishwasher. All appeared to be in good repair and tested operational. LPA observed a two day supply of perishable food and and a seven day supply of non-perishable food along with a sufficient amount of emergency water.

LPA observed all resident rooms to be clean and free of any debris and hazards, all rooms appeared to be large enough to walk about freely and for the bedrooms to have all required furnishings including a lamp, a chair, a bed with clean linens in good repair; meaning no strains or tears, a chest of drawers, and enough storage space to store personal belongings. LPA observed all restrooms to be clean and free of any mildew and debris. LPA observed all faucets and toilets to be operational. LPA observed restrooms to have all required furnishings such as grab bars, slip resistant floor matts, and a shower chair. Hot water temperature tested between 105.4 and 107.1 Degrees Fahrenheit.
