Facility Evaluation Report

Licensing Program Analyst (LPA) Hanna Gough made an unannounced visit to the facility to conduct a case management visit. LPA was greeted and granted entry by Licensee Vivian Ruedas and discussed the purpose of the visit. Licensee informed LPA that they would like to close the facility.

LPA toured the facility and observed no residents in care.

The last resident left the facility on February 2, 2025.

LPA requested the facility license as it is part of the closure process, and the license was surrendered during the visit. LPA informed the licensee of the licensing procedures for future facility operation if desired.

An exit interview was conducted and a copy of this report was provided at the end of the visit.
