The inspector’s account

Licensing Program Analyst (LPA) Hanna Gough made an unannounced visit to the facility to conduct a Plan of Correction (POC) visit in regards to citations issued during the required annual inspection on March 25, 2026. LPA was greeted and granted entry by staff. LPA met with Direct Support Professional (DSP) Daisy Lucero and discussed the purpose of the visit. DSP attempted to contact Administrator (AD) Diana Hernandez by phone and did not get a response.

LPA reviewed client files and reviewed 3 of 3 completed files at the facility. Therefore, citation 80070(a) has been fulfilled.

Based on today's observations no deficiencies were noted per Title 22 Division 6 of the California Code of Regulations. An exit interview was conducted and a copy of this report along with clearance letter was left at the facility.
