The inspector’s account

Licensing Program Analyst (LPA) Hanna Gough made an unannounced visit to the facility to conduct a Plan of Correction (POC) visit for deficiencies issued on March 25, 2026 during the required annual inspection. LPA arrived at the facility and there was no answer at the door. LPA called Administrator (AD) Diana Hernandez and discussed the purpose of the visit. AD informed LPA that they cannot come to the facility at present time. LPA was greeted and granted entry by staff. LPA met with Direct Support Professional (DSP) Albert Trujillo.

LPA reviewed staff files and observed all required elements were present, thus clearing citation 80066(e). LPA printed the clearance letter and provided it to the facility at the time of the visit.

LPA reviewed client files and did not observe completed client files with items missing such as physicians reports and TB tests for 3 of 3 clients. LPA also did not observe an updated Individual Program Plan (IPP) for Client #2 which resulted in a failure to correct civil penalty for deficiency 80070(a) of $100 a day until corrected.

Based on today's observations a civil penalty is being noted per Title 22 Division 6 of the California Code of Regulations. An exit interview was conducted and a copy of this report along with LIC421FC, LIC 858, LIC859 and appeal rights were left at the facility.
