Facility Evaluation Report
Licensing Program Analyst (LPA) Hanna Gough made an unannounced visit to the facility to conduct the required annual inspection. LPA was greeted and granted entry by staff. LPA met with Direct Support Professional (DSP) Daisy Lucero and discussed the purpose of the visit. DSP contacted Administrator and Licensee Diana Hernandez and informed DSP that they could not meet LPA at the facility due to training and granted DSP permission to assist LPA with the visit.
The facility is a one story home with four client bedrooms, one staff bedroom, two bathrooms, living room, dining room, kitchen and detached two car garage. The facility currently has three clients in care. LPA observed the client bedrooms to have the required components and furnishings. LPA observed a clean supply of linens in the hall closets located by the client bedrooms by the laundry room. LPA observed the client bathrooms to be stocked with soap, toilet paper and paper towels. LPA tested the water to be at 106.3 degrees Fahrenheit. LPA observed the kitchen to be clean and free of vermin. LPA observed a two day perishable and seven day non perishable food supply on hand. LPA observed all appliances to be operational. LPA observed the emergency food and water supply to be in the food pantry located in the laundry room. LPA observed the fire extinguishers in the kitchen to be charged and with a service date of October 1, 2025. LPA observed the toxins and chemicals to be in a locked cabinet located in the closet by the staff bedroom. LPA observed the knives and sharps to be in a locked cabinet located in the closet by the staff bedroom and separate from the toxins and chemicals. LPA observed a shaded seating area in the backyard for client use and is free of debris and obstructions. DSP tested all fire alarms and they were found to be operational. DSP tested the carbon monoxide detector and it was found to be operational.
