Facility Evaluation Report
Licensing Program Analyst (LPA) Jerome Haley conducted a case management visit regarding observations and discoveries made during the investigation into complaint control # 22-AS-20250630094024
During the complaint investigation mentioned above, LPA Haley made observations and discovered information that requires a citation and/or a technical advisory that can turn into a citation if not address in a timely manner.
During interviews with two individuals, it was discovered the camera in the backyard is not working.
While in the backyard making observations, the fence line that separates the facility from the public park directly behind the facility was observed to be in need of repair or reinforcement in a couple different areas along the fence line. S1 explained there’s been ongoing negotiations with the City and/or Edison to put up a new fence. S1 explained since the electric pole is located inside the property line, the city need to approve the construction to the fence.
S1 admitted the facility did not submit an incident report to the Regional Office regarding the theft of an Electric bike that belonged to a client. S1 confirmed an incident report was sent to the Regional Center (RCOC), the family of the client was notified, but S1 admitted they forgot to notify Licensing (CDSS CCLD ASCP).
As a result of today’s Case Management visit, deficiencies will be cited and a technical violation will be issued for the observations and discoveries made during the complaint investigation. Photos were taken.
An exit interview was conducted and a copy of this report, LIC809D, LIC9102, and appeal rights were provided.
