Facility Evaluation Report
Licensing Program Analyst (LPA) Jerome Haley conducted an unannounced visit for the purpose of conducting a required one-year annual inspection. LPA was greeted, granted entry by staff and explained the reason for the visit.
The facility is a single level structure and licensed for six ambulatory clients. All clients were present at the beginning of the visit and a couple of them left for their day program shortly after the visit started. There’s a total of 7 bedrooms for the clients, and two of the bedrooms are vacant and being used as a storage space. There are two restrooms available. There’s a living room space, a dining space, backyard area, and an attached garage. Bedrooms: All bedrooms have the required furnishings: bed, lamp, chair, and storage space. Bathroom(s): Bathrooms are equipped with a working toilet, wash basin, and shower. No chemicals observed in the bathroom. Hygiene items were locked below the sink in the main bathroom. Kitchen: 4 of 4 burners are operational on the gas stove. Sharps are kept locked in a cabinet next to the stove. Cleaning chemicals stored in a locked cabinet directly under the sink. Food Service: A supply of perishable and non-perishable food items that meet regulation requirements was observed. Emergency food was observed in on of the vacant rooms, and emergency water was observed in the garage.
Client & Staff Files : Client and staff files stored in a locked closet next to the front door.
File Review: 3 of 5 client files were reviewed during the visit, and four staff files were reviewed.
Medications/First-Aid Kit: Client medications are stored in a locked closet next to the front door. A first aid kit with all the required elements was observed in the locked closet, and a disaster kit was observed.
