Facility Evaluation Report

Licensing Program Analyst (LPA) Jerome Haley made an unannounced visit to conduct a case management visit regarding an incident that took place on Friday, March 13, 2026.

LPA Haley was greeted, granted entry and explained the reason for the visit.

During the visit, LPA Haley was led on a tour of the facility. The facility was clean and well organized. A perishable and nonperishable food supply that meets regulations was observed.

A Health and Safety check was conducted and 2 clients were observed. Both appeared to be happy, healthy, and well taken care of.

At this time the incident needs further investigation.

An exit interview was conducted, and a copy of this report was provided.
