Facility Evaluation Report
On today's date Licensing Program Analyst (LPA) William Vanegas made an unannounced visit for the purposes of conducting an annual inspection. Upon arrival LPA was greeted and granted entry to the facility by direct support staff. Administrator (AD) Melba Santos was notified via telephone and arrived shortly after to assist with the annual inspection. LPA began a tour of the facility accompanied by AD and observed the following. The AD has a valid Administrator certificate that is valid from July 1, 2024 through June 30, 2026.
The facility is a one storied home with six bedrooms which are all utilized for clients in care, two bathrooms, and a detached two car garage. LPA observed kitchen area to be clean and free of any mildew and debris. LPA observed the kitchen area to have a gas stove, refrigerator, and a microwave all appeared to be in good repair and tested operational. LPA observed a two day supply of non perishable food and a seven day supply of perishable food, as well as a sufficient amount of emergency water on hand.
LPA observed all client rooms to be large enough to walk about freely, and for the bedrooms to have all required furnishings such as a chair, a lamp, chest drawers, a bed, clean linens in good repair; meaning no strains or tares, and enough storage space to store personal belongings. LPA observed all bedrooms to be free of any debris and hazards. LPA observed all restrooms to be clean and free of any mildew and debris. LPA observed all faucets and toilets to be operational and in good repair, LPA restrooms to have all required furnishings such as grab bars, slip resistant floor matts, and a shower chair if needed. Hot water temperature in the restrooms tested between 112.1 and 114.3 degrees Fahrenheit.
