The inspector’s account

Licensing Program Analyst (LPA) Hanna Gough made an unannounced visit to the facility for the purpose of conducting the required annual inspection. LPA was greeted and granted entry by staff. Staff contacted Administrator Marisa Hernandez and explained the reason for the visit. Administrator gave Caregiver (CG) Andrew Nudo permission to assist LPA with the inspection and that they will be available by phone. The facility currently has four clients of which two are at day program and one went on an outing.

The facility has four client bedrooms, one staff bedroom, two bathrooms, a kitchen, living room, laundry room, dining area, and detached two car garage. The facility appears clean, safe, and sanitary. The staff bedroom is locked making it inaccessible to clients in care. The kitchen appears clean and sanitary. LPA observed that the facility has a two day supply of perishables and a seven day supply of non-perishables. LPA observed the emergency food and water supply located in the kitchen. LPA observed the knives and sharps to be locked in a cabinet next to the kitchen making them inaccessible to clients in care. LPA observed medication to be locked in a cabinet located next to the kitchen making them inaccessible to clients in care. LPA observed that client bedrooms had all the required furnishings but client bedroom two, three, and four do not have window screens. Administrator was able to provide a work order for window and screen repair at the time of the inspection. LPA observed the client bathrooms to have toilet paper, paper towels, and non-slip mats for the showers. LPA observed the water temperature reading at 147.0 degrees Fahrenheit in the client bathroom located by the front door and 145.4 degrees Fahrenheit in the bathroom located by the back door. LPA observed two fire extinguishers charged with a service date of April 2025. LPA observed the backyard to have a covered seating area for client use and is free of obstructions. LPA observed toxins and chemicals to be stored in a locked cabinet in the backyard making them inaccessible to clients in care. LPA tested fire and carbon monoxide detectors and they were found to be operational.
