Facility Evaluation Report
Licensing Program Analyst (LPA) Hanna Gough made an unannounced visit to the facility to conduct the required annual inspection. LPA was greeted and granted entry by staff. LPA met with Administrator (AD) Rosario Dacayo and Licensee Imelda Samonte and discussed the purpose of the visit. The facility currently has three clients in care of which all are on outings or at day program.
The facility is a one-story home with four client bedrooms, three bathrooms, staff room, living room, kitchen, dining room, and attached garage. The facility appears clean, safe and sanitary. LPA observed the client bedrooms to have all the required components and furnishings. LPA observed the client bathrooms to have paper towels, toilet paper and non slip mats in the shower. LPA tested the water to be between 111.9-116.4 degrees Fahrenheit. LPA observed the staff room to be clean with no medications accessible to clients in care. LPA observed the kitchen to be clean and free of vermin. LPA observed a two day perishable and seven day nonperishable food supply on hand. LPA observed the knives to be in a locked drawer located by the stove and made inaccessible to clients in care. LPA observed the toxins and chemicals to be under the locked kitchen sink and made inaccessible to clients in care. LPA observed all appliances to be operational. LPA observed a fire extinguisher in the kitchen to be charged and with a service date of September 29, 2025. LPA observed the emergency food and water supply to be located in a cabinet in the dining room. LPA observed the client centrally stored medications to be in a locked cabinet located in the dining room making them inaccessible to clients in care. LPA observed a clean supply of linens in a hall closet located by the client bathroom in the hall by the dining room. LPA observed the backyard to be free of debris and obstructions. LPA observed a shaded seating area for client use. LPA observed the garage to be clean and used for extra storage. LPA and staff tested the fire alarms and smoke detectors and they were found to be operational.
