Facility Evaluation Report
Licensing Program Analyst (LPA) Jerome Haley arrived to conduct an unannounced Case Management visit regarding information the facility self reported to the Regional Office. LPA Haley explained the reason for the visit upon entering the facility. 69 residents were present at the time of the visit, of which 25 are resident’s housed in the Memory Care unit.
During the visit, LPA was led on a tour of the memory care unit with staff. During the tour of the Memory Care unit, a Health and Safety check was conducted. R1 was observed in their room sleeping with the blankets pulled up to the residents’ chin area. Only the resident’s face was exposed. R1 appeared to be resting peacefully and appeared healthy looking in the face.
LPA requested relevant documents regarding Resident 1 (R1), Staff 2 (S2), Outside Staff 1 (OS1), and Resident 2 (R2). Documents were also requested for two additional caregivers regarding a previous investigation. LPA requested documents be sent via email.
A resident and staff roster was provided during the visit. S1 was advised additional documents may be requested.
No deficiencies are being cited as a result of today’s Case Management visit.
An exit interview was conducted, and a copy of this report was provided.
